To be eligible for the 2026 season, the League must verify the following documents:

1. Physical Copy of Birth Certificate (Must bring to assessments to be verified in person if you are a new player to our league)
2. Medical Release Form signed by parent/guardian (Bring this form to assessment day)
3. 3 Proof of Residency documents OR School Enrollment Form (these should be uploaded to your profile online OR bring to assessment day for verification).
4. Photo Release Form
