Document Verification - Jan 10th 2026

To be eligible for the 2026 season you must bring all of the following documents to assessments for verification:

*New players that did not play last year, bring the following:

1. Paper Birth Certificate for age verification check

2. Medical Release form signed by Parent/Guardian (attached below)

3. 3 proof of residency documents OR school enrollment form (if not previously uploaded to player profile)

4. Photo Release Form

*Returning players; bring the following:

1. Medical Release form signed by Parent/Guardian (attached below)

2. 3 proof of residency documents OR school enrollment form (if not previously uploaded to player profile)

3. Photo Release Form

NOTE: if you provided a school enrollment form last year and your child did not change schools, then you do not need to provide again.

** players that are not in a league age requiring assessments can bring documents for verification anytime between 9-11:30am** 

Please find copies and helpful links below:

1. Physical Birth Certificate (Must be original. Cannot be a copy, we will not be keeping it)

1. Medical Release Form signed by parent/guardian (Bring this form to assessment day)

2. 3 Proof of Residency documents OR School Enrollment Form (these should be uploaded to your profile online OR bring to assessment day for verification).
